SLIS Forms and Tips

• Outside Course Approval Form
  MIS students may count up to 6 hours outside SLIS toward the MIS degree. Specialist (post-master's) degree students may count up to 15 hours outside SLIS with approval. These outside courses must be approved by the student's adviser.
• Course Waiver Approval Form
  If course waiver is approved, the number of hours required for the degree remains the same. The waiver allows students the option of an extra elective course rather than a required core course.
• S605 Application and Guidelines (IUB)
  Details on internship opportunities are available. Final approval must be obtained through Howard Rosenbaum.
• S601 Directed Readings
  Allows for students to study a specific field of specialization in which there is not enough interest to warrant regular classroom instruction. The approval process is on the SLIS Course Schedule page - by semester.
• S602 Directed Research
  Allows for students to develop or conduct research projects under faculty supervision. These research projects are often extensions of work begun in a course, or in some other way build on preparation received during studies at SLIS. The approval process is on the SLIS Course Schedule page - by semester.
• Audit Form
  Most SLIS classes are only available for credit. In general "participating" courses - particularly ones with labs - are not available to audit at SLIS. However, check with the course instructor if interested. Use the Registrar's "Memorandum for Course Audit" form to sign up.
• Ph.D. Forms

  Proposal deadlines are:
  January 31 for summer & fall courses
  September 30 for spring courses

• Computer Access for Guests - (in addition to the official UITS request, faculty may request access to SLIS computers by emailing the SLIS technology staff.)

• Graduation Application (Masters & Specialists)
  This form is for SLIS degree processing. Students should submit forms to the SLIS Office (Room 011) for May, June and August graduation by a preferred date of the first week of February; and for December graduation by the first week of September. Ph.D. students should see thePh.D. Contact page
